PERMISSION FOR TRANSPORTATION
1) I authorize and give permission for my child(ren) to be transported to Timberlane Arts and Dance Academy LLC for music/dance lessons.
2) I agree to release and hold harmless Timberlane Arts and Dance Academy LLC from any liability to me or my child which may arise as a result of my child being transported.
3) I will notify Timberlane Arts & Dance Academy by ten a.m. if my child is not riding the can on the scheduled day.
4) If my child does not wear his/her seat belt, he or she will not be permitted to continue to ride the van and no refund will be given.
5) My child understands that he or she is not to leave any garbage or trash in the van.
6) On teacher planning days and early release days I understand that TADA does not pick-up on the those days after school.
